Learning Objectives

After completing this continuing education activity you will be able to:

  1. Identify how an ICM intervention provides support to prevent hospital readmission.
  2. Describe the primary outcome and methods of the ICM pilot quality improvement (QI) project.
  3. Summarize the results of the ICM pilot project and recommendations for future initiatives to support patients at risk of readmission.

Learning Outcomes

Seventy-five percent of participants will be able to demonstrate knowledge of intensive case management (ICM) to reduce hospital readmissions by achieving a posttest score of 70% or greater.
