Learning Objectives

After completing this continuing education activity you will be able to:

  1. Explain how factors related to Covid-19 impacted behavioral health readmission rates.
  2. Identify the social determinants of health (SDoH) that effect behavioral health readmission rates.
  3. Describe the study results of readmission rates when comparing outcomes from implementing the care management intervention versus treatment as usual (TAU).

Learning Outcomes

Seventy-five percent of participants will be able to demonstrate knowledge in case management interventions to address determinants of health for individuals with multiple behavioral health readmission by achieving a posttest score of 70% or greater.
