  2. Describe the methods and results of the quality improvement (QI) project designed to reduce 30-day hospital readmissions.
  3. Identify the authors' QI project outcomes and interventions that were used by discharge planners for reducing 30-day hospital readmissions.

Learning Outcomes

Seventy-five percent of participants will be able to demonstrate knowledge of targeted discharge planning for high-risk readmissions focused on patients and caregivers to reduce 30-day hospital readmissions by achieving a posttest score of 70% or greater.
