After completing this continuing education activity you will be able to:

  1. Identify features of the Agency for Healthcare Research and Quality's (AHRQ's) IDEAL discharge planning strategy and the American Association of Retired Persons' CARE Act.
  2. Analyze caregiver perceptions of discharge teaching using the framework of the IDEAL strategy.
  3. Select postdischarge complications related to caregiver knowledge gaps.

Learning Outcomes

Seventy-five percent of participants will be able to demonstrate knowledge on implications for transitions from hospital to home by including caregivers in IDEAL discharge teaching by achieving a posttest score of 70% or greater.
