To present a quality improvement (QI) project to improve trauma service referral compliance to an existing home-based elderly fall prevention program through the implementation of a daily management system (DMS).

Learning Objectives/Outcomes

After completing this continuing education activity you will be able to:

  1. Summarize the issue of falls among community-dwelling older adults and the current status of a Community Paramedicine Fall Prevention Program (CPFP3).
  2. Outline the methodology utilized to measure whether implementation of a DMS would increase trauma service referrals to the CPFP3.
  3. Specify the results, conclusions, and limitations of the project.
