Learning Outcomes

Seventy-five percent of participants will demonstrate knowledge of a study's findings on the association of implementing an interdisciplinary team educational intervention on post-hospital transitions of care and the impact on post-acute expenditures by achieving a minimum score of 70% on the outcomes-based posttest.
