Learning Objectives

After completing this continuing education activity you will be able to:

  1. Describe the rates, risk factors, and impact of community-acquired pneumonia (CAP)-related hospital readmissions on health care systems.
  2. Identify evidence-based pneumonia care interventions endorsed by the Infectious Diseases Society of America/American Thoracic Society (IDSA/ATS).
  3. Recognize the outcomes of an evidence-based, case manager-led pneumonia care bundle at SNFs on 30-day hospital readmissions for pneumonia.

Learning Outcomes

Seventy-five percent of participants will be able to demonstrate knowledge of 30-day hospital readmission outcomes in a study implementing a case manager-led pneumonia care bundle in skilled nursing facilities (SNFs) by achieving a posttest score of 70% or greater.
